You don't upload that one as far as I know. When your lvl 5 put in your username and start quest 1 which is to reach lvl 12. That's when you upload your image and you will have to click threw a few things saying that you agree and know to upload the quest info like has to be full game, Has to be exactly the requirements stated eg since quest 1 is lvl 12 you can't upload the image with lvl 11 or lvl 13 has to be at lvl 12.

The game will automatically notify Bananatic that you are level 5 so you can begin quests.

I'm sorry, but don't you mean level 14? I don't see level 12 as a quest requirement.
